An initial report into the tragic accident during parachute training over Saskatchewan is focused on the human factors that may have lead to the incident.

Master Corporal Alfred Barr was taking part in search and rescue technician training when he died in March in the Yorkton area. Investigators say his parachute did not open properly, causing him to go into a spiral.  Witnesses say he started making movements like he was trying to untangle his lines before hitting the ground.

The flight safety report is intended to prevent a similar incident from happening.
